If your child attends any extra-curricular activities on school premises that run during our operational hours, you will need to click and complete this form before your child can commence the activity.

Medication Authority Form

If your child is required to take any ongoing or temporary medications eg; ADHD medication or a course of antibiotics during our operational hours, please download and complete this form.
