If your child is required to take any ongoing or temporary medications eg; ADHD medication or a course of antibiotics during our operational hours, please download and complete this form.
As Part of our OOSH program, we occasionally offer movie sessions to create a relaxed, educative and enjoyable environment for children.
Educational program opportunities or other circumstances may see us screen PG rated movies during OOSH hours. We kindly request your ongoing permission for your child to participate in PG rated viewing experiences for the duration of their enrolment at the service, unless this permission is withdrawn in writing.
Details of the movies can be provided upon request.
PLEASE NOTE: Please fill out one form per child. All movies shown at the service will be age appropriate and suitable for children

To protect the privacy and safety of all children, families and visitors are not permitted to take photographs or videos while on service premises. This includes images or recordings of your own child.
All NWCC families must download and complete this form before commencing OOSH.
If your child attends any extra-curricular activities on school premises that run during our operational hours, you will need to click and complete this form before your child can commence the activity.
4 Steps Required For Attendance
PLEASE NOTE:
• If Children go home straight from the Extra Curricular activity with their guardian,
NWCC requires advanced permission in writing.
• ** If NWCC needs to look for your child at the end of their activity,
then a penalty fee will be charged.
• When Children are signed out of OOSH,
they are the responsibility of the Extra Curricular Provider, not NWCC.
• NWCC is unable to deliver children to and from Extra Curricular activities.
