Direct Debit Form

All NWCC families must download and complete this form before commencing OOSH.

Extra-Curricular Form

If your child is booked into OOSH and is also booked into another outside activity during this time, you will need to complete an online permission form each term.

This form needs to be completed by the Primary or Secondary Guardian on the NWCC Enrolment Form.

4 Steps Required for Attendance
  1. Complete the Extra-Curricular Form each term.
    Children may not attend this activity until approval is granted by NWCC in writing.
  2. Children must first sign into OOSH prior to leaving for their activity.
  3. The External Activity Provider is responsible for the collection and return of your child to and from the external activity.
  4. Advise NWCC of any changes to your child's extra-curricular arrangements.

PLEASE NOTE:

  • If children go home straight from the Extra Curricular activity with their guardian, NWCC requires advanced permission in writing.
  • If NWCC needs to look for your child at the end of their activity, then a penalty fee will be charged.
  • When children are signed out of OOSH, they are the responsibility of the Extra Curricular Provider, not NWCC.
  • NWCC is unable to deliver children to and from Extra Curricular activities.

Getting to Know Your Child Form

As part of the Re-Enrolment process, all families are required to complete the Getting to Know Your Child Form for each child enrolled with NWCC.

The information provided will form part of your child's enrolment record and assist our team in providing the best possible care and support. Please complete a separate form for each child to secure your child's enrolment.

Medication Form

If your child is required to take any ongoing or temporary medication, such as ADHD medication or a course of antibiotics, during our operational hours, please download and complete this form.

Photo & Video Consent Form

To protect the privacy and safety of all children, families and visitors are not permitted to take photographs or videos while on service premises. This includes images or recordings of your own child.

All NWCC families must complete this form before commencing OOSH.

PG Rated Movie Permission Form

As part of our OOSH program, we occasionally offer movie sessions to create a relaxed, educational and enjoyable environment for children.

Educational program opportunities or other circumstances may see us screen PG rated movies during OOSH hours. We request your ongoing permission for your child to participate in PG rated viewing experiences for the duration of their enrolment, unless permission is withdrawn in writing.

Details of movies can be provided upon request.

Please note: Please complete one form per child. All movies shown at the service will be age-appropriate and suitable for children.

Regular Outings Authority Form

Occasionally, children may take part in supervised activities within the school grounds, including planned activities, drills or relocations required for safety or operational reasons.

This form is required annually. Please submit a separate form for each child.
